Second Student Technology Needs Survey Available

 

If you have a Pre-K -12th grade student currently enrolled in VISD who does not have a device that they can use to access online instruction, or does not currently have home internet access, and you were unable to fill out our previous device survey, please fill out the new survey here: This survey is no longer available; closed April 1.

 

Please do not fill out this survey if you have previously filled out the device survey or have called and had a staff member fill out the survey for you, or if you called and were put on a waiting list.

 

After filling out the survey, if your student is eligible to receive a device or internet hotspot, a campus Librarian will call you to schedule a pickup time. Please be advised that due to inventory limitations, there may be an extended delay between survey completion and device pickup.

 

If you were unable to fill out the survey or have transportation concerns, please call 361-788-2828 to speak with a staff member who will complete the form for you, schedule your time slot, and help address your concerns.

 

After 5:00 pm on Wednesday, April 1st, the survey will close, and no further calls for devices will be accepted. We will not be opening another survey for devices after this one.

 

REASONS YOU MAY NOT RECEIVE A DEVICE:

  • If you fill out this second student device survey and have not received a call to schedule your pickup time by Wednesday, April 8th, then you were not eligible to receive a device or hotspot for your student. If you have questions about this, please call 361-788-2828.
  • If you filled out the previous student device survey and have not received a call to schedule your pickup time by Wednesday, April 1st, then you were not eligible to receive a device or hotspot for your student. If you have questions about this, please call 361-788-2828.
  • If you selected “no” to the last question in the survey, indicating that you do not agree to the terms you will be asked to follow, you are not eligible to receive a device or hotspot for your student.
  • If you refuse to sign the agreement upon device pickup, you will also be denied a device and / or hotspot, and will be required to either provide a device for your student or pick up a paper packet from their campus to continue their education, or their grade-level promotion may be at risk.

Please be advised that you will be required to pay a minimum of $20 for each device and accessory that you either return to us damaged, or that has been stolen or lost. If the device has been stolen, the district must be provided with a police report of the incident within 10 business days, otherwise the device will be deemed “lost” and you will be required to pay the district for the full price of the device. A cost table associated with these charges will be located on the agreement you will receive and be asked to sign upon device pickup.

 

The “Technology Lending Agreement” posted below has been updated to reflect these changes. All costs in the cost table on that agreement are for example only and are subject to change.
